VACCINATION DOCUMENATION

ASB is excited to partner with Attend Safe to provide secure and HIPAA compliant vaccine verifications. Safety is our top concern and we have taken the steps necessary to model the gold-standard of onsite health and safety protocols for the upcoming events.
 
As part of the BakingTECH 2022: Baking Evolution event and City of Chicago requirements, you will be using the Attend Safe web-based application for the submission of your vaccination card/records for validation (HIPAA secure verification). We encourage you to access the web-based application below, as soon as possible, to submit your vaccination records. This will ensure a smooth registration process on the day of the event.

HOW TO SUBMIT VACCINATION DOCUMENTATION

  2. Complete your registration/requested information
  3. Click on proof of vaccination 
  4. Submit your vaccination card for verification
  5. Receive your verification email and/or request for additional information 

HOW TO SUBMIT DOCUMENTATION FOR A MEDICAL OR RELIGIOUS EXEMPTION

For those attendees who are not vaccinated but have a medical and/or religious exemption from vaccination, you are still required to submit your documentation via the Attend Safe application. There will be two steps to your verification process. As a first step, you must upload your medical/religious exemption documentation for review and approval/verification through the web-based application. Your medical/religious exemption must clearly state your full name, reason for exemption, date of exemption and authorize professional providing the exemption. We encourage you to access the web-based application below, as soon as possible, to submit your vaccination records.

  3. Click on medical/religious exemption
  4. Submit your medical and/or religious exemption documentation for verification
  5. Receive your verification email and/or request for additional information 
For your second step, you MUST submit a COVID-19 test result within 72 hours of arrival at the event. Once you receive your test results (clinically administered between February 27 – March 1, 2022) you must submit your test results through the attend safe web-based application by following the following steps:

  3. Click on negative COVID-19 test
  4. Submit your medical and/or religious exemption documentation for verification
  5. Receive your verification email and/or request for additional information 

WHAT CONSTITUTES ACCEPTABLE DOCUMENTS?

Acceptable Vaccination Documents:
  • Actual photo of your CDC issued vaccination card clearly showing your full name, vaccination type received, date of shot(s) and administering facility/clinician.
  • Medical records (EMR) including your full name, vaccination type received, date of shot(s) and administering facility/clinician.
  • To be considered fully vaccinated, you must have your one dose and/or your second dose (based on vaccination type) two weeks prior to your arrival day in Chicago.
  • We do not accept QR codes from state run applications and/or Clear.
 
Acceptable COVID-19 Medical/Religious Exemption Documents:
  • Photo of your medical/religious exemption documentation clearly show your full name, reason for exemption, date of exemption and authorize professional providing the exemption
 
Acceptable COVID-19 Test Documents:
  • Photo of your negative COVID-19 test results clearly showing your full name, type of clinically administered test, date of test and administering facility/clinician. Acceptable tests for the ASB BakingTECH 2022 EEvent is a clinically administered rapid antigen or PCR. 
  • Medical records (EMR) of your negative COVID-19 test results clearly showing your full name, type of clinically administered test, date of test and administering facility/ clinician. Acceptable tests for the ASB BakingTECH 2022 event is a clinically administered rapid antigen or PCR. 
  • We will not accept self-administered over the counter test results.
  • Test results must be clinically administered within a 72-hour window of your arrival on-site at the Hilton Chicago, IL. COVID-19 tests administered prior to 72 hours will not be verified and attendees will be required to submit to on-site testing provided by Attend Safe.
